Patch to merge ImageSourceCairo.cpp and ImageSourceSkia.cpp

Some notes:

1) Please read the ChangeLog in the patch carefully for a detailed list of the
changes.
2) I am not able to build either Skia or Cairo as I'm not set up for it,
however I've been very careful in the refactoring and gone through it
line-by-line.
