Decided to combine the tiering patch with implementing dynamic optimization,
because it turns out that my efforts to get dynamic optimization working
revealed the need to make some changes to tiering heuristics.  This patch is
not in reviewable state yet, because it depends on a value profiling patch that
is still in the commit queue.
