Another round of revisions.  This version is known to build under Cairo on
Windows, but this patch does not touch several other files that need to be
updated (RenderThemeSafari, PlatformScrollBarSafari).

I would really like to get this first series of changes committed before
attacking the other stuff outside platform/graphics.
