After inspecting my original message, I may not have been clear enough, sorry.

When opening the app in Safari on my Mac, I can allow access to the camera. After clicking on the camera icon in the url I can also always allow the access. This works fine.
On my iPhone, I do not see the same option. When clicking on the camera icon in the url bar I can revoke the access, but I do not see an option to always allow. 
When installing the app to the Home Screen as a PWA, I also have no option to always allow access. 

I hope this clarifies the issue.
