I have some additional information. It looks like the issue is related to the Cross Origin Opener Policy header. 

We can repro the issue 100% of the time when COOP is set to "same-origin-allow-popups". No repro when it is set to "unsafe-none" (the default). 

I'm happy to set up two test pages on subdomains with the header set to each of these values, if that is helpful.
