We also starting seeing this issue in iOS 14.2 and have been getting a lot of complaints about it recently. It occurred randomly, maybe 1/10 times. Video was working but the user on the iOS Safari 14.2 device could not hear the other. Our workaround was to play the audio stream from the MediaStream audio track through the AudioContext.
