Hey @Dan - I'm on Anna's team. We haven't found a way to reliably reproduce the issue. It seems to happen at random. That said, the issue does seem to happen somewhat frequently - maybe 10% of the time.

We haven't found any special conditions that need to exist in order to reproduce the issue. It appears to happen when the mediaStream is attached to an audio element in the DOM. If I detach then re-attach an existing audio track to the DOM, it can trigger the issue about 10% of the time.
