I have just installed macOS 10.4.4 with Safari 12.1 on a fresh partition. I went to https://packfrog.com/test.php and clicked on the link. I then ended up with an image named willi.png in my Downloads folder.

It appears I am not able to reproduce the issue you report with your test case. Any idea what I am doing differently?
