I found a way to reliably reproduce the issue: Pin the tab. Unpinning the tab will make it go away again.

I have not been able to reproduce the issue an any Mac yet that does not have pins (n=6), would be interested to hear if anyone found a counter example.

(Found in a Reddit thread from 6 months ago where Protonmail support mentions this a known issue with Safari, and it's already been reported to Apple.)
