We don't appear to have any tests related to decode failures. As an example, we didn't catch http://crbug.com/125802.

The tests should load various files that cause decode failures and ensure that the correct error is reported along with other correct states. Creating the various media files will be the hardest part of writing this test. We should aim for the first, a middle, and maybe the last frame to cause decode errors. An audio decode error at similar points would be nice as well.
