https://bugzilla.mozilla.org/show_bug.cgi?id=41924 is worth a read. Joe's patch seems to be in the spirit of http://hixie.ch/specs/alttext rather than current Firefox treatment of broken images, which does not display a broken image icon at all.

Is it only lack of testcases blocking Joe's patch now? Benjamin, would you r+ it with more testing?
