Perhaps the misunderstanding is that this is about the interaction between WebKit (where it is obviously possible to make text be italic by introducing the right markup) and the Cocoa user interface - in other words, hook up a standard "Format" menu to the webview, and does this work as you would expect?  

I think that comment #10 is good analysis -- since it's possible to see synthesized italics in a webkit application, and since you can choose command-I in Safari (which, I presume, had to work around things a bit), it would make sense for other client applications to make text italic as well, without having to re-invent the wheel.
