This patch incorporates some of Darin's comments.  Unfortunately, it fails http/tests/navigation/back-send-referrer.html, perhaps because that test calls a History API from an inactive document.  I need to investigate (1) whether that's why the test fails and (2) what other browsers do in this situation.
