Miscellaneous minor fixes / improvements:
  - numerous style fixes
  - numerous unnecessary headers
  - addDatabase no longer needs a bool return value, no need to check for an exception
  - addDOMStorage no longer needs a bool return value, ditto
  - added a few "const"s
  - fix DatabaseTableView's statusbar icon
