Revised patch

Revised patch:
- added QtPluginGraphicsWidget::create()
- added QWebPagePrivate::graphicsWidget
- made FrameLoaderClientQt::createPlugin() use QWebPagePrivate::graphicsWidget
as the parent item

I second Simon that moving that plugin code where it belongs could be a
separate commit.
