commit-queue rejection messages should include setters name

Without looking at bugzilla's edit history I can't tell who set the
commit-queue+ bit.  I think it makes sense to record that in rejection
messages.

Example text: "Rejecting Eric Seidel's request for auto-commit.  Patch failed
to download and apply."
