r3453 of the HTML 5 spec added support for a "*" entry in the online whitelist
section of an HTML 5 application cache. Presence of a "*" entry indicates that
all URLs are automatically whitelisted. See
<http://lists.whatwg.org/pipermail/whatwg-whatwg.org/2009-July/021242.html> for
more justification.

We should implement support for "*" in the online whitelist of an HTML 5
application cache.
