I can't tell how to reproduce it for sure, except that it happens pretty
frequently for me in development. It happens for a co-worker of mine as well.
It seems to happen most often, though not necessarily, when creating images
with javascript. I've also seen it refuse to load new versions of images added
by javascript. Clicking the Empty Cache... menu item doesn't help. I've still
only seen this problem on localhost, incidentally.
