I don't have any test cases available at the moment. The website where this
happens most often is http://forum.osnn.net/. Which is a website I am
administrator for. It also happens on eCollege.net which is used by my
university to provide us with an online class shell/way to contact instructors
and turn in assignments.

Osnn.net is currently running:

Apache 2.2.9
PHP 5.2.6

Not entirely sure if that is part of the puzzle, or if the problem is somewhere
else. Once again, when I get some free time I am going to check this out for
myself, and at that time I will hopefully also have a test case to test it
against.
