http://vanguard.com. An attempt is made to get online documentation on a new
fund addition. The link if for a pdf. When the link is clicked, a popup is
produced to the Vanguard Forms & Literature area where you are then asked: 
Click Yes to add your name and address. Then you can fill out more of this form
online.
Click No if you want to fill out the form online using different information or
if you want to print it blank. 
The "Yes" and "No" and "Cancel" form buttons do not work.
