Source of form.

NOTE: The problem was triggered by the first form containing a "Watch this
Item" submit button.
There is another "Watch this Item" link earlier in the document, and another
"Watch this Item"
submit form later in the document; I didn't experiment with those, and do not
know their outcomes.
