So in your reduction, it seems we never actually hit the
"isLoadingFromPageCache()" case.  I think we need to split off part of the BF
list management into a seperate method here, to distinguish between the back
forward LIST and the back forward CACHE - we still have to keep the list
consistent with what we'd expect
