This is almost definitely caused by
http://trac.webkit.org/projects/webkit/changeset/20813, which fixed
http://bugs.webkit.org/show_bug.cgi?id=12005.  Ironically, that bug involved
*too many* history items getting put in.

This bug, however, sounds a lot like
http://bugs.webkit.org/show_bug.cgi?id=3546 (which was fixed in
http://trac.webkit.org/projects/webkit/changeset/20878).

In that bug, gmail was found to be opening its new window in an extremely
peculiar way that precluded the creation of that initial history item.  It
seems likely that something similar is happening here.
