When CreateStream is called after a plugin makes a request for content
(NPN_GetUrl), the PluginData is gotten using the transaction ID.  If the
PluginData exists, CreateStream will return.  PluginData can be created inside
CreateStream, if called from returning content from the construction of the
plugin.  It can also be created by the plugin requesting additional data.  This
needs to be handled correctly.
