Not sure if this adds anything useful to the discussion, but I'm seeing this
problem in my Rails app running on lighttpd.  I'm just using a plain multipart
form with a file upload, and every so often, Safari's request just seems to go
missing enroute to the server.  I'm not seeing anything in the server logs to
indicate that the request was received, and Safari just sits there waiting for
a reply, and eventually times out.
