It's a known shortcoming (in Radar already) that if Safari has been to a site
and then the site icon changes, Safari doesn't always notice the existence of
the new icon. (This happens because Safari caches the old icon, or even the old
lack-of-icon, to avoid rechecking on every page load.) So that might be what
happened here.

Deleting ~/Library/Safari/Icons will reset all of Safari's knowledge about
favicons, and thus the next time you visit a site with a favicon Safari should
pick up the favicon.
