Patch

Updated patch. I think I've addressed most of the comments. I haven't added the
comments pointing to the spec in the command definitions yet, but I haven't
forgotten, it's just that I haven't had time and I wanted to submit a new patch
to get feedback for all other updates. I'll continue tomorrow.
