I believe this address all the comments on the previous versions.

Note that this version includes a small yak shave that may belong in a separate
patch (removing some duplicate code in creation of NetscapePluginStreamLoader
and making it behave more like its sibling, SubresourceLoader).  Let me know if
you'd prefer I break it out.
