Reviewed by Yusuke Suzuki and Yijia Huang.

Using LazyNeverDestroyed avoids the bespoke singleton code for now
(though eventually we will need a custom solution to handle the
sequestering constraint), and moving the once_flag to the cpp file
avoids duplication per TU.
